I think Ursa killed Azulon and ran away. I also think Ozai must've known about it or taken some kind of part in it because Azulon's dying (Since it was DYING, he must've been killed with a poison or something, but people don't know how to detect poison so they probably thought he died of old age) wish was for Ozai to be the Fire Lord.

I don't think so actually I think Ozai made that up. He probably asked for them to leave them alone and azulon died in that time. not being witnesses he said that his father's dying wish was for him to be fire lord.
